This sample South Carolina Heal Care Power of Attorney form allows an individual (the "Principle") to grant certain rights to another or others [the "Agent(s)"] to individually or collectively make health care decisions in accordance with the Principle's indicated wishes (selected checkboxes or statements) on behalf of the Principle during, and only during, such time as the Principle is incapacitated and is unable to make such decisions for himself or herself.
